Description

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Works with the operations to find improvement ideas on processes that improve the quality of care provided to our patients through programs that provide financial incentives and technical assistance to support our provider network.
  1. Review and understand Pay for Performance (P4P) programs for different health plans.
  2. Follows up on weekly P4P action plans with all CHSI locations.
  3. Compile, sort and verify the accuracy of data from all CHSI locations before it is entered.
  4. Maintains all P4P program records.
  5. Performs monthly P4P audits of each health plan.
  1. Analyzes and communicates results of the P4P program to necessary parties.
  2. Collects opportunities and updates list accordingly.
  3. Audits the usage of the P4P program.
  4. Identifies opportunities for improvement within the clinic environment on the P4P program.
  5. Trains and coaches team on P4P program, CHSI initiatives and process to capture P4P data, and best practices to make the P4P process effective and efficient.
  6. Gather and analyze data to solutions for return on investments
  1. Perform all other duties as directed either formally or informally, verbally or in writing.
Requirements

 

KNOWLEDGE, SKILLS AND ABILITIES:

  • Strong organizational skills and attention to detail; ability to manage multiple responsibilities simultaneously.
  • Strong electronic health records (EHR) system experience; proficiency with Nextgen preferred. · Strong computer skills, with proficiency in Microsoft Office
  • Well-developed verbal and written communication skills and the ability to work cooperatively with divergent groups
  • Must possess great customer service including verbal and written communication.
  • A high standard of professionalism and professional ethics and conduct is expected in speech, manner, attitude and appearance at all times.
  • Possess strong interpersonal skills and ability to work well with others.
  • Ability to deal effectively with changing situations and stressful environment.
  • Ability to perform tasks related to physical activity to complete the responsibilities of the position.

 

EXPERIENCE AND EDUCATION:

  • High School Diploma or equivalent.
  • Minimum of six (6) months experience in a similar clerical role with solid knowledge of electronic health record (EHR) system; preferably in a healthcare setting and Nextgen.
  • Knowledgeable in healthcare related incentive programs such as pay for performance (P4P).
Summary

The Continuous Improvement Coordinator is responsible for the assistance of collecting, inputing, analyzing and reporting on processes that improve the quality of care provided to our patients through programs that provide financial incentives and technical assistance to support our provider network. The coordinator will work collaboratively with the Quality Improvement, Operations and Business Development Departments in making sure all program deliverables are met timely, executing program operations, and helping guide and implement re-occurring and new program initiatives in line with our health plan requirements.
