SUMMARY

In your role as Community Education and Staff Development Educator you will report to the Chief Patient Experience and Compliance Officer. Your responsibilities will include developing detailed action plans, facilitating meetings, and effectively communicating and documenting final results of all projects. In addition, you will create and maintain the ongoing design of education programs and plans based on sound clinical evidence/guidelines.

DUTIES
• Develops and delivers health education programs in response to community and clinic needs. Topics will include, but are not limited to, HIV, STDs, Viral Hepatitis, Diabetes, and High Blood Pressure
• Identifies or develops tools for and implements activities to deliver heath outreach and education. Identifies locations, events, and other strategies and activities to promote education of targeted audience
• Assist in outreach education by participating in community events as needed to help educate our community on healthy lifestyles (i.e. – nutrition, HIV, diabetes
• Develops trusting relationships with community members and clinic patients, creating an appropriate space to provide health education for all age ranges
• Coordinates and delivers health education and promotion to community groups or patient populations as needed
• In collaboration with clinic providers, monitors clinic patient adherence to care plans, provides health education, and encourages patient to engage in care plan
• Provide training support for other health professionals and CHWs

EDUCATION AND EXPERIENCE REQUIRED
• Bachelor’s degree
• Previous experience in an education position
• Previous experience using and Electronic Health Record
• 5 years as a Registered Nurse
• Current CA RN License
• Current Instructor level American Heart Association BLS
• Competency with Microsoft Office and other computer programs
