In your role as Health Management Technician, you will report to the Health Management and Education Manager. Your responsibilities will include assisting physicians, and nurse practitioners in the care of patients in a family practice/primary care environment.

Duties

  • Perform chart prep activities per current checklist in advance of patient visit to ensure complete records available
  • Assist in ensuring previous test results and procedure results are present and documented accurately in the EMR
  • Welcome patients by greeting them in person or on the telephone
  • Verify patient information by interviewing patient, recording medical history, confirming purpose of visit
  • Follow established protocols for assisting patients and clinicians with varying patient visit types
  • Perform preliminary physical tests, including but not limited to; take blood pressure, weight and temperature; document patient history summary
  • Evaluate need for and perform appropriate health screening and questionnaires
  • Record and reconcile current medications and allergies
  • Document all information accurately and completely in the electronic medical record.
  • Notify provider of needed screenings and assist provider with patient examination or procedure as directed
  • When appropriate, provide, monitor, log and document patient immunizations as ordered by the provider, and within the scope of MA practice. Ensure two-person verification of all immunizations prior to giving and signing off
  • As directed by provider, provide educational information to patients and instructions, answering questions
  • As directed by provider, and following established protocols, facilitate patient connection to a Community Health Worker when needed
  • Clean and restock patient examination rooms between patient use in compliance with infection control standards
  • Assist with follow-up on referrals and communicate with referrals team
  • Follow-up on the status of all post-visit orders; communicate with the patient to assist with completion of orders if needed. If a Community Health Worker is assisting the patient, liaise with them to ensure coordinated patient approach
  • Keep equipment operating by following operating instructions, maintaining supplies and reporting issues
  • Secure patient information and maintain patient confidence by completing and safeguarding medical records, keeping patient information confidential

Education and Experience required

  • High School Diploma or equivalent
  • 2 years experience assisting patients in a clinical setting
  • Successful completion of a Medical Assistant training program recognized by the state of California
  • Certification/Registered Medical Assistant
  • BLS (American Heart Association)
