Job Summary

The Comprehensive Prenatal Health Worker I (CPHW I) provides support services, ongoing orientations, assessments, care plan development, appropriate nutrition, health education and psychosocial interventions and referral when needed, to UCHC’s prenatal patients.

 

Essential Functions

  • Conducts face-to face interviews with all prenatal patients, screens patients for presumptive eligibility (PE) and issuing of PE card if eligible; Issues the 2nd good thru extension for patients.
  • Identify opportunities and facilitate efforts to expand service or increase patient access into
  • Assures client charts are documented whenever telephone & face-to-face contact is
  • Codes all super bills accordingly to type of visit and signs all chart entries using legal signature and
  • Provides ongoing orientation, assessments, classes and makes the appropriate referrals when needed to our prenatal
  • Prepares all Intake packets English/Spanish and makes sure all appointments have been given to patient i.e., Intake, Initial, Medi-Cal, etc.
  • Assist patients in completing Medi-Cal forms (application) and follow-up on no show/missed
  • Orders all prenatal forms as needed and fax weekly PE enrollment Summary Forms to the
  • Fax weekly schedules to Medi-Cal worker and advocate for patients with the Medi-Cal
  • Case management of the prenatal program, which includes reviewing all prenatal charts after each clinic visit with the provider, as well as reviewing each chart the day before the visit with the provider. This is to assure that all procedures, tests, etc. that the provider has ordered have been done and lab results have been received on a timely manner.
  • Attends recommended job-related trainings provided by the CPSP County

Qualifications

  • High school diploma or GED Medical Assistant (MA) certificate preferred.
  • Required: One (1) year experience in a community health center

Physical Requirements and Environmental Conditions

Work Schedule

  • You will generally be scheduled to work each day Monday through Friday, with starting times each day varying between 8:00 a.m. and 9:00 a.m. and ending time between 5:00 p.m. and 6:00 p.m., as required to meet the operational needs of the company. There will be occasional Saturdays as needed.
