<Job Overview>
We are seeking a compassionate and dedicated Community Health Worker to join our team. The ideal candidate will play a vital role in connecting community members with essential health services, promoting health education, and supporting individuals in navigating the healthcare system. This paid position offers an opportunity to make a meaningful impact on community wellness through outreach, education, and personalized support.

<Duties>

● Educating members about services, assisting them with enrollment and serving as the primary liaison between the members and any services they may need.

● Support individuals and family as they navigate the healthcare system and transition to improvement in self-care and health care management.

● Helping bridge conversations with members and remove barriers that prevent them from accessing health and social services; and conduct face-to-face outreach to panel of members for appointment scheduling, needs assessment, and care gap closure.

● Meeting members in clinic, facility or at home to help identify social determinants of health impacting member’s health and general well-being.

● Assist members in accessing health-related services and community resources, such as accompaniment to specialist appointments and assistance with enrollment forms.

● Document interactions with members and on behalf of members in medical record.

● Knowledgeable about community resources appropriate to needs of patients/families.

● Always maintain HIPAA compliance.

● Serves as the bridge between the Patient and the healthcare system.

● Focuses support on Patients who are difficult to engage and/or who have cultural or linguistic barriers to care via field-based contacts including accompaniment to appointments.

● Help patients connect with transportation resources and provide appointment reminders in special circumstances.

● Assists the ECM Care Team in engagement efforts of eligible ECM Patients in the clinic and with the community.

● Assists Patients in navigating the healthcare system and community resources.

● Follows-up by phone and in person with eligible ECM Patients and patients enrolled in ECM.

● CHW is responsible for engaging with Patients, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as Motivations Interviewing) and promotes collaboration between the Patient and his or her medical/behavioral team.

● Engaging those eligible but not yet enrolled as well as those currently enrolled in Med-Cal to re-engage for follow-up after discharge.

● Distribute health promotion materials.

● Advocates on behalf of the Patients with health care professionals.

● Other duties as assigned.

<Qualifications>

  • Valid California Driver’s License.
  • High school graduate or equivalent required.
  • Prior experience or training is highly desirable.
  • Knowledge of working with people with disabilities and understanding of medical terminology is essential.
  • Strong communication skills for public speaking, teaching, and community engagement activities.
  • Ability to collect data systematically and maintain detailed records of client interactions.
  • Demonstrated ability to work effectively with diverse populations in a respectful and inclusive manner.
  • Written and oral fluency in English and Spanish is preferred.

Job Type: Full-time

Pay: From $23.21 per hour
