The HIV and PrEP Care Coordinator will coordinate patient access to PrEP medication, HIV testing, education, linkage to care treatment, and counseling resources. This role will work in partnership with community-based organizations, patient assistance programs, behavioral health, and primary care providers to support patient care.

Schedule: M-F 8am/8:30am-5pm(30 minutes or 1 hour lunch). This is a hybrid position(24 hours onsite and 16 hours remote). 

Responsibilities

  • Conducts outreach and motivational interviewing to pre-screen and enroll patients at risk for HIV in PrEP medication and adherence programs
  • Coordinates referrals and access to PrEP program visits, medication adherence education, and other resources such as counseling and testing services, including home rapid tests
  • Supports outreach efforts to increase patient awareness and participation in routine HIV testing and PrEP program enrollments
  • Conducts HIV test education and assists with test result counseling to support linkage to HIV care treatment
  • Connects newly HIV and Hepatitis C diagnosed patients with health education, access to follow up visits, and support services,
  • Assists with collaborative support resources in partnership with community partners, local government programs, and Neighborhood services
  • Identifies patient needs and refers to available risk reduction programs
  • Assists in data collection to evaluate and improve outcomes for outreach, education, and PrEP medication adherence efforts
  • Participates in analysis of programs and assist with operational improvement efforts
  • Attends and participates in required meetings, trainings, and offsite community events

Qualifications 

Education/Experience

  • High school diploma/GED required; associate’s degree preferred
  • Three years of experience in a healthcare-related field required
  • Experience working within LGBTQA+ and other HIV high-risk communities preferred
  • HIV Test Counselor and HIV prevention certifications and trainings preferred
  • Medical Assistant certification preferred
  • Experience in eCW or electronic medical records preferred
  • Bilingual (English/Spanish) highly preferred
  • Valid CA driver’s license and proof of car insurance required

Additional Qualifications (Knowledge, Skills and Abilities)

  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with educating others about HIV, PEP, and PrEP
  • Ability to empathize and sympathize with a high level of sensitivity to multi-cultural communities
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner

Physical Requirements

  • Ability to lift/carry 25 lbs/weight
  • Ability to stand for long periods of time

Pay range: $27.21 – $38.09 per hour, depending on experience.

Compensation Disclosure: The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
