Health Education Specialist II – Community Engagement Lead
(Extra-Help)
The Health Equity Program (HEQ) of San Bernardino County Department of Public Health (DPH) is
seeking a passionate and dedicated Health Education Specialist (HES) II – Community Planning and
Engagement (CE) Lead to play a vital role in advancing health equity and fostering meaningful
connections within our community. This position will lead outreach efforts, collaborate with diverse
organizations, and design impactful health education initiatives that empower individuals and promote

Role: Assist in implementing various health education, community assessment, planning, and
community health improvement activities. Duties may include:
Community Education & Engagement
• Identify and assess community education needs, including those identified in the Community
Health Improvement (CHI) process.
• Plan, develop, and evaluate required health education programs and community engagement
activities.
• Organize and engage existing community groups, organizations, and leaders.
• Build relationships with external organizations to assess health needs, gaps, and priorities.
• Facilitate outreach events, community forums, and presentations in various locations.
• Provide direct health education and outreach efforts (e.g., community fairs, presentations).
Collaboration & Partnerships
• Foster cooperation and participation among community-based organizations and agencies to
support Community Health Improvement Plan (CHIP) goals and strategies.
• Conduct one-on-one meetings with external partners.
• Identify areas of potential partnership and connect external partners to internal programs and
SBC Community Vital Signs Initiative.
• Participate in partner and coalition meetings to support public health and CHI objectives.
Planning & Program Development
• Develop operational plans to achieve community engagement and community health
improvement objectives.
• Plan, coordinate, and oversee community health forums.
• Facilitate events and meetings, including monthly collaborative sessions and subcommittee
meetings to support CHIP implementation.
• Plan, design, and prepare educational materials (pamphlets, posters, flyers, media releases).
• Determine barriers to optimal health among priority populations to identify outreach
locations and strategies.
Data & Reporting
• Collect and review data related to health outcomes and social drivers
• Develop community forum summaries to document key themes, trends, demographics, and
recommendations.
• Develop Final Comprehensive Community Health Forum Summary Report.
• Compile and report on data from the County’s Community Transformation Plan
• Complete and update partner relationship documentation (e.g., 1:1 sheets).
• Submit bi-weekly reports related to daily tasks and engagement goals and CHI activities.
• Complete CE reports with metrics for data reporting.
• Update the CE and HEQ team calendars as needed.
Team Supervision
• Lead and manage the community engagement team by distributing responsibilities effectively
and ensuring successful execution of program initiatives, including CHI activities and CHIP
implementation efforts.
• Oversee the planning and coordination of community health forums conducted as part of the CHI
process.
• Conduct regular one-on-one meetings with CE team members and for progress updates on
CHIP related activities.
• Evaluate staff effectiveness, track program outcomes, and ensure timely completion of
community forum summaries and reporting requirements.
• Ensure adherence to County and DPH policies and standard practices.
• Maintain team records and make improvements based on data.
• Process timesheet submissions and leave applications.
• Report on team efforts during program meetings.
Operational & Compliance Responsibilities
• Maintain current knowledge of health education, community engagement, and community
health improvement best practices.
• Ensure privacy and confidentiality protections.
• Maintain a HIPAA-compliant workspace.
• Conduct policy and procedural reviews to support program compliance.
• Maintain care of district-assigned tabling equipment and outreach materials.
• Complete vehicle logs and send reports after each use.
• Foster a collaborative environment within HEQ, and across DPH programs, especially, the
Community Vital Signs (CVS) to support CHI and CHIP goals, and Health Promotion and
Education Services (HPES).
• Maintain ongoing communication with program leadership (Supervising Health Education
Specialist, Program Coordinator, Program Manager) regarding progress on community health
forums and CHIP implementation.
• Perform additional duties as assigned.

Conditions of Employment
Travel: Travel throughout the County may be required to perform duties off-site as needed. At the
time of hire, a valid California Class C driver’s license and proof of automobile liability insurance must
be produced for the individual providing the transportation.
Background Investigation: Incumbents selected for these positions must pass a background
investigation, which includes verification of employment history and education as well as a physical
exam and drug screening.
Shift Availability: Incumbents may be required to work occasional evenings and/or weekends
depending on assignment.
Employment Status: Extra help positions are in the Unclassified Service, which serve at the discretion
of the appointing authority and do not attain regular status. These positions participate in the County’s
Part-time Seasonal Temporary (PST) Deferred Compensation Plan and do not earn any other
benefits.
Disaster Service Worker: Positions in DPH are designated Disaster Service Workers and may be
scheduled to work after-hours, weekends and holidays to cover emergency response.
Minimum Requirements
Applicants must qualify for one of the following options:
Option A – Education and Experience:
Education: A Masters degree in Public Health (MPH) with emphasis on Health Promotion and
Education, Community Health, Health Administration Public Health Policy, Global Health,
Environmental Health, Health Sciences, Nutrition, Epidemiology, or closely related degree*
-AND-
Experience: One (1) year of full time equivalent experience in public health in a public health
department, health services provider organization, healthcare organization, or community based
health organization, which included the following: planning implementing and evaluating health
education programs conducting community health education activities, developing social media
campaigns and instructional educational materials, organizing events, and working with
community coalitions and developing community partnerships.
Option B – Education and Experience:
Education: A bachelor’s degree in Public Health, Health Promotion and Education, Community
Health, Health Administration Public Health Policy, Global Health, Environmental Health, Health
Sciences, Nutrition, Epidemiology, or closely related degree*
-AND-
Experience: Two (2) years of full-time experience as stated in Option A
Desired Qualifications
The ideal candidate will possess a master’s degree in a qualifying field of study and will possess
the Certified Health Education Specialist (CHES) and/or the Master Certified Health Education
Specialist (MCHES) certificate. Additionally, the ideal candidate will possess excellent written
and verbal communication skills as they relate to developing education materials and media
releases, representing the program in public, and conducting presentations and training. Similarly,
the ideal candidate should also have experience in community organizing or advocacy,
demonstrating the ability to engage stakeholders, foster partnerships, and drive initiatives that
promote health equity and social justice.
