  1. Upholds and supports the mission, philosophy, objectives, policies and ethics of CHSI.

  2. Provide timely feedback to Director of Community Relations (DCR) and work in collaboration with the Community Relations Supervisor (CRS) to implement the Community Relations Department’s specific strategic plan to promote healthy eating, active living.
  3. Meets regularly with the Clinic Administrator and designated staff to help identify and/or communicate community issues, feedback, service needs, events, and the Community Relations Department’s plan to engage providers and staff in support of Healzone strategies and outcomes.
  4. Comply with expectations of grant requirements for the purpose of statistical tracking to generate monthly reports. Use Electronic Medical Records as a tool to assess chronic illness management and improvement.
  5. Serve as a liaison between the Community Relations Department, health center staff, and the community while continuing contact with social networks and community partners.
  6. Uses prospecting methods to generate community partnerships including initiating personal/business contacts, sourcing expansion opportunities through existing and new accounts, cold calling and educational presentations.
  7. Provide and support patients with resources, education, and referrals for the purpose of assisting patients with their chronic disease management.
  8. Attend key community meetings and events as a representative of assigned clinic, for the purpose of networking to build contacts, incorporation of HealZone strategies and community change outcomes.
  1. Ability to find resources to assist patients with the needs outside the scope of our services at CHSI.
  2. Acts as a liaison in performing value-added activities, customer service and service recovery to ensure patient satisfaction.
  3. Continuously update the designated Community Relations Coordinator that administers CHSI educational social media campaigns.
  4. Perform all other duties as directed either formally or informally, verbally or in writing.



  • Bilingual (English/Spanish) strongly preferred
  • Strong analytical and critical thinking skills; ability to pay close attention to detail.
  • Strong computer skills, with proficiency in Microsoft Office
  • Well-developed verbal and written communication skills and the ability to work cooperatively with divergent and diverse groups
  • Must possess good customer service skills including verbal and written communication.
  • A high standard of professionalism is expected in speech, manner, attitude and appearance at all times.
  • Possess strong interpersonal skills and ability to work well with others.
  • High standard of professional ethics and conduct is expected.
  • Ability to deal effectively with changing situations and stressful environment.
  • Ability to perform tasks related to physical activity to complete the responsibilities of the position.
  • Be able to lift up to 50 lbs. with the help of another person.



  • Three years or more of related experience required. Bachelor Degree or five years of experience preferred.
  • Current CA Driver’s License and proof of automobile insurance
  • Must be able to travel locally 60% of the time

The Community Health Educator supports the clinic(s) by the implementation of the Community Relations Department’s objective to carry out the mission of CHSI through developing community relationships, creating and aligning programs that promote health and wellness, targeting at-risk populations, serving as a clinic to community resources liaison, working in alignment with Riverside HealZone strategies, and educating the community about healthcare system navigation and social determinants of health.
