Introduction

               The Community Health Association Inland Southern Region (Association) is a champion
               for community-based health centers and clinics located in Riverside and San Bernardino
               Counties. We provide services to support our diverse membership in providing access to
               care and quality health services to the most vulnerable communities. The strategic work
               of the Association is in concert with the organization’s mission and vision:

               Mission Statement
               We exist to support our membership and community partners to ensure accessible, high
               quality, and cost-effective health services.

               Vision Statement
               We envision health care access for all and efficient use of health services and resources
               for the Inland Southern Region.

               The  Association conducted its  fourth  annual membership satisfaction survey  during
               October,  November  and December  of  2020.  The information  was  gathered  using
                               ®
               SurveyMonkey ,  an on-line computer based program, and  provided insight into  the
               membership’s opinions and beliefs with the Association’s performance, responsiveness
               and support to member’s needs, as well as suggestions for service delivery. The data
               collected from the annual membership survey was aggregated and will be used to inform
               future services and trainings provided by the Association.

               Participating sites included community health centers and clinics licensed as Federally
               Qualified Health Centers (FQHC’s), government and free clinics, community clinics, and
               hospital-based clinics  in the Association’s membership. All organizations in the
               Association’s membership are licensed entities that serve vulnerable communities.

               Overall  the  Board of Directors  indicated they are satisfied with the staff and services
               provided. A participation rate of 100% was achieved. This specific survey consists of data
               gathered from the Board of Directors for the year 2020. To view the data and trends
               throughout the four  years  (2017-2020), please see Appendix B.  The  next annual
